Why Age Spots Turn Darker Before They Fade: What Happens in the Two Weeks After Laser or IPL
That coffee-ground crust on a treated spot is not a burn or a failed session. Here is the biology of pigment clearance, what a normal timeline looks like, and how to tell healthy darkening apart from post-inflammatory hyperpigmentation.

One of the most common calls a dermatology office gets after a pigment session goes something like this: the spot I paid to remove is now darker than it was before. For most patients treated with q-switched or picosecond lasers, or with intense pulsed light, that darkening is not a complication. It is the expected first stage of clearance. Understanding why requires a quick look at where the pigment in an age spot actually sits.
A solar lentigo, the technical name for a classic age spot, is a patch of skin where chronic ultraviolet exposure has pushed melanocytes to overproduce melanin and hand excess pigment to the surrounding keratinocytes. That pigment is concentrated in the epidermis, the outermost living layer of skin, which fully turns over roughly every 28 to 40 days depending on age. This turnover cycle is the engine that carries treated pigment out of the skin, and it explains almost everything about the post-treatment timeline.
What the light actually does. Lasers and IPL devices used for lentigines target melanin through selective photothermolysis. Melanin absorbs specific wavelengths of light far more strongly than the surrounding tissue does. When a short pulse hits the spot, pigment-laden cells absorb the energy, heat rapidly, and are damaged while neighboring structures are largely spared. Picosecond devices add a photoacoustic effect, essentially shattering pigment into smaller fragments with pressure waves rather than heat alone.
Why it darkens. Within minutes to hours, the treated spot often turns gray, then distinctly darker brown. Clinicians sometimes describe the endpoint as ashen whitening followed by darkening that resembles coffee grounds. Two things are happening. First, damaged pigment-containing cells oxidize and cluster near the surface. Second, the injured epidermis begins pushing that debris upward as new keratinocytes form beneath it. The darkened material is, in effect, pigment being packaged for disposal. On the face, this micro-crust typically flakes off in 5 to 10 days. On the hands, forearms, and chest, where epidermal turnover is slower and circulation is less robust, expect 2 to 3 weeks.
The normal timeline, in plain terms. Day 0 to 1: the spot looks darker, sometimes with mild redness or a frosted gray tone. Days 2 to 7: a thin, dry crust forms and the spot may look worse in photos than the untreated baseline. Days 7 to 21: the crust releases on its own, revealing pink or slightly lighter skin underneath. Weeks 4 to 8: the pink fades and the true result becomes visible. Judging a pigment treatment before the one-month mark is like judging bread halfway through baking.
When darkening is actually a problem. The look-alike to watch for is post-inflammatory hyperpigmentation, usually shortened to PIH. This occurs when the inflammation from treatment itself stimulates melanocytes to produce new pigment, and it behaves differently from healthy crusting. PIH tends to appear 2 to 4 weeks after treatment, not immediately. It is often larger or blurrier than the original spot, may look brown-gray rather than crisp brown, and it does not flake off because it is fresh pigment sitting in living skin, not debris on the way out. PIH risk rises sharply with darker Fitzpatrick skin types, recent tanning, aggressive fluence settings, and sun exposure during healing. It usually resolves over 3 to 12 months, and topical agents such as azelaic acid, tranexamic acid, or hydroquinone can shorten that course, but prevention is far easier than correction.
What helps the process along. The evidence-backed list is short. Strict broad-spectrum sunscreen, SPF 30 or higher, reapplied when outdoors, because ultraviolet light directly stimulates the same melanocytes you are trying to quiet. A bland moisturizer or petrolatum to keep the crust flexible, since a hydrated wound bed re-epithelializes faster than a dry one. And patience: no picking. Mechanically removing the crust early can trigger the very inflammation that causes PIH, and in deeper injuries it can leave a pale scar that is far harder to fix than the original lentigo.
What does not help. Scrubbing, retinoids, exfoliating acids, and vitamin C serums applied to freshly treated skin do not speed pigment clearance and can irritate the healing epidermis. They have a legitimate role in maintenance after healing is complete, roughly 2 to 4 weeks out, but not during the crusting phase.
One more caveat worth taking seriously. Any spot that darkens should have been correctly diagnosed before it was treated. Lentigo maligna, an early form of melanoma, can closely mimic a benign age spot, and treating it with a cosmetic laser can mask a lesion that needed a biopsy. A spot with irregular borders, multiple shades, recent change, or a size above 6 millimeters warrants dermoscopic evaluation before any light-based treatment. The darkening described in this article is normal biology, but it should only ever happen to a lesion a clinician has already confirmed is benign.
The short version: temporary darkening after laser or IPL is the visible proof that pigment is being escorted out through normal skin turnover. Give it a full month on the face, longer on the body, protect it from the sun, and let the crust decide its own departure date.
